TheFormTool is fussy about the layout of the Q&A Table -- it looks for particular information in particular places, and when things are rearranged it gets confused.
This form has a ton of formatting changes to the QAT -- it's very impressive. And formatting changes generally do not cause problems. It's only changes in layout that cause problems. In this form, there are two layout changes that confound the program:
1. The first two rows in the QAT need to be the TFT copyright row and the Label/Question/Answer row. In this form, the ENTRY OF APPEARANCE row needs to be moved below the Label/Question/Answer row, so that TFT will recognize it as a Divider.
2. The Label/Question/Answer row needs to be three columns. By combining those three cells into a single cell, TFT no longer recognizes it.
So, the general rule is: There is lots of room for formatting in the Q&A Table, as this great example shows, but don't mess with the headers. TheFormTool is very protective of the first two rows and will almost always react badly to any modifications.
