The attached Word 2010 document shows how to build a completely personalized six-factor identification number. While this example uses two dates and a last name to build a completely unique number sequence, any number of other factors can be plugged into it for a similar effect. A particularly interesting approach uses the built-in "Now" feature to include a date stamp as part of the numbering sequence.

The end result should be a completely unique number that also imparts useful information to the system or person eventually using it.
